libapache2-mod-form software package provides Apache module to decode data submitted from Web forms, you can install in your Debian 8 (Jessie) by running the commands given below on the terminal,
$ sudo apt-get update $ sudo apt-get install libapache2-mod-form
libapache2-mod-form is installed in your system.
Make ensure the libapache2-mod-form package were installed using the commands given below,
$ sudo dpkg-query -l | grep libapache2-mod-form *
You will get with libapache2-mod-form package name, version, architecture and description in a table
